Sonja Nelson is the Community Programs Coordinator at Michael E. Debakey VA Medical Center in Houston, Texas. Ms. Nelson’s position is located within the Veteran’s Care and Transitional Service(VCTS) care line, where she serves elderly and disabled veterans and their families by utilizing an array of services offered through the Veteran Health Administration’s (VHA) geriatrics and extended care (GEC) programs.
Sonja holds a Master of Social Work from Chicago State University and is a Licensed Clinical Social Worker. She has been employed in the field of Social Work over the past 20 years, with much of her time spent working in the area of Direct Practice with Children and Families. Over the past 14 years, Sonja has focused predominantly on working with demographic groups and communities that have been historically underserved and disproportionately marginalized. These areas of practice have included geriatrics, mental health, corrections, and healthcare, serving the uninsured/underinsured.
Sonja’s work with these groups has impacted individuals and families in a variety of settings including the private sector, community agencies(local and state-supported), and her current role with the Department of Veteran Affairs, Veterans Health Administration(VHA).
In her current position as the Community Programs Coordinator, Sonja assists veterans and their families in accessing long-term services and supports that allow elderly and disabled veterans the opportunity to age in place in their homes and communities surrounded by loved ones.
